I'm sure you are all aware how important it is for our office to have all the emergency information for your child. This includes contacts if you cannot be reached in an emergency, allergies, medical conditions, names and phone numbers of your Doctors, Dentists and Orthopedists. Everyone needs to fill out the "Emergency Form" which is located on the website under the "Parents" tab as soon as possible. This year, we will be using the Resurrection school web site to collect some forms that were previously submitted on paper. To access these, you will need a login to the web site. Each year we print a directory of our families, also known as the ?Blue Book? or ?Family Directory?. We would like to include your Family Information in this directory which we will send home to families who have paid their PTO dues. This will hopefully make correspondence easier for the Class Mom?s, Committee Heads and the P.T.O.
